Financial Policies
As a courtesy, we will be happy to file an insurance claim on your behalf. However, there are several administrative steps that are required prior to your surgery:
- If your insurance requires a written referral from your Primary Care Physician, it is your responsibility to obtain it.
- We will verify your insurance benefits prior to your surgery.
- If there are any outstanding insurance deductibles and/or co-insurance, you will be required to meet those obligations prior to undergoing your procedure.
- We may not be contracted with all insurance companies. If this is the case, your “out of network” benefits will apply.
- After the insurance claim is processed, we will bill you for the balance on your account.
- Co-pays, coinsurance, and/or deductibles will be your responsibility whether in or out of your insurance network.
- You are responsible for the cost of any testing or lab services performed by external facilities and billing for such services may be submitted independently by physicians at the facility.
- If you have a balance on your account, you are required to pay the balance in full prior to being seen by one of our providers.
- We accept Cash, Checks, Debit Cards, Care Credit, Google and Apple Pay.
Credit Card Payment Surcharge
For our customers who choose to pay with VISA, DISCOVER, AMEX or MC, cardholders will be responsible for the merchant transaction fee. To offset the processing fees associated with credit card transactions, our practice implements a 3% surcharge that is passed through to merchant services on payments made using Visa, Mastercard, American Express, or Discover credit cards. This surcharge is applied to the total transaction amount and is not applied to debit card transactions. This practice aligns with industry standards and complies with card network regulations, which stipulate that surcharges must not exceed the actual cost of processing and must be clearly disclosed to the customer.
